How to request your medical records

  2. Include your name, date of birth, hospital number and other information you consider relevant
  3. Attach proof ID identification (e.g., drivers licence, passport)

About Subject Access Requests (SAR)

You have a right to access the medical records we hold about you
The right of access, commonly referred to as subject access request (SAR), gives individuals the right to obtain a copy of their personal data, as well as other supplementary information. 

Is there a fee?
There is no charge for making a SAR.

How quickly do you have to respond?
We we have received proof of your identity, we will respond to you within one month.

Providing evidence of your identity
You must provide evidence of your identity when you make a request with I.D documents (e.g. passport or drivers licence).
