Pain
How you managed any pain or discomfort before you became unwell:
- Did you have any regular pain or aches?
- How did you manage it or treat it?
- Did it affect what you could do or how you felt?
Respiratory Function
How well your lungs are working, your breathing patterns, any oxygen support you may need, and how your breathing affects your daily life.
Cognitive Capacity
How your thinking, memory and learning are working now. Any changes from before, and how well you can focus and take in new information.
Nutrition & Swallow Needs
What you eat and drink, how well you can chew and swallow safely, your weight and general health, and any support or advice you need with food and fluids.
